The e-Renewal system will not allow you to change your status online. If you need to change your license status, you will be required to mail your renewal application and fee to the department for processing. You may download the the Reactivation Application form from the Board’s website, “Resources” Tab or send a request to the Board office at: mqa.occupationaltherapy@flhealth.gov
