You can view, confirm, or make changes to the information that will be published in your practitioner profile by logging in to the Practitioner Login page. In carrying our legislative mandate to publish practitioner profiles, we want to ensure the information that we publish is accurate. Accordingly, we ask that you please review your profile for any changes, corrections, and/or omissions. If you see the statement ”The practitioner did not provide this mandatory information”, please provide that information. We will not accept curriculum vitae or resumes in place of you providing specific information. Changes, excluding education and training, year began practicing, and liability claims, can be made to your profile electronically by following the instructions below. Please note that Section 456.042, Florida Statutes, requires practitioners to update profile information within 15 days after a change of an occurrence in each section of your profile. Attention Newly Licensed Practitioners Chapter 456.041(7), Florida Statutes (link), requires you to submit changes to the department within thirty (30) days from receipt of notice. If you do not make changes within thirty (30) days, your profile will be automatically published. Once you have completed your review and made any necessary corrections, click on “Confirm Changes”. The Practitioner Confirmation Page will display the information that will be published online, at which time you must “Confirm” the profile again before the changes will be implemented. Note: Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.
If you apply online, there is a form that you must print out that will have your ORI number pre-populated. In the paper application, you can locate your ORI number in the instructions.
The department will mail a renewal notification to your last known address at least 90 days prior to the expiration of your license. Your license also indicates the date it will expire. All OT and OTA licenses expire on February 28, of each odd year.
If a PDF file fails to download, shows up blank, or freezes your internet browser, the file was most likely corrupted during the download process. To correct the problem, you will need to first clear your browser’s cache, then close and restart the browser, which entails logging back into MQA Services to continue downloading the document. If the cache is not cleared the cached version of the document, which is likely damaged, will still appear.
During an open renewal period, if your license is in Clear/Inactive status, to reactivate to Clear/Active status you must pay a $160 ($100 reactivation fee + $60 Renewal fee). You must also submit online in CE Broker, the required 26 hours of continuing education for the renewal biennium you changed your status to Inactive and 26 hours for each licensure biennium, in which the license remained in an Inactive status. If you are reactivating your license outside of a renewal period, an additional $50 change of status fee is required.
If your license is in a Delinquent/Active or Delinquent/Inactive status, additional delinquent fees will apply. You will need to contact the Customer Care Center (850) 488-0595 or email the Board office at: mqa.occupationaltherapy@flhealth.gov for specific requirements.
These transactions cannot be completed online. These fees and changes must be completed by mail. You must submit the Reactivation Application form (download from the Board’s website, “Resources” tab or send a request to the Board office at: mqa.occupationaltherapy@flhealth.gov). See Rule 64B11-5.006 Inactive and Delinquent Status Fees.
You must renew your license every two years and pay the current renewal fee. The Board will continue to contact you regarding renewals. Inactive status does not allow you to continue providing therapy. However, all licensees accrue continuing education requirements, in an and and inactive status. You may continue to renew inactive without having to submit continuing education requirements, however, when you choose to reactivate to an active status you must complete and submit proof of all continuing education (26 hours), for EACH renewal biennium you have been inactive, and pay the $50 change of status fee and $100 reactivation fee.
You must submit a written request to convert your retired license to an active status. To reactivate the license you must pay the same amount as the renewal fee $60 for an active status licensee for each biennial licensure period in which the licensee was in retired status. You must also submit proof of completing 26 hours of CE, in CE Broker (www.CEBroker.com) including the statutory mandated courses (2 hours laws and rules and 2 hours medical errors) for each biennium the license was in a retired status.
You must submit a request to place your license in "retired" status and pay a one-time fee of $50.00 plus the $5 unlicensed activity fee, for a total fee of $55. The Board will no longer contact you regarding your license. Retired status does not allow you to continue providing therapy.
Yes. Board Rule 64B11-6.001 (4)(b), Florida Administrative Code, states courses sponsored by an approved accredited (American Occupational Therapy Association (AOTA) www.aota.org) college or university when providing a program/curriculum for occupational therapists or occupational therapy assistants shall be awarded 10 hours of continuing education credit per semester hour and shall be verified by official transcripts.
The Board’s measurement for Continuing Education hours is by contact hours. One (1) contact hour equals a minimum of fifty (50) minutes. One half (1/2 or .5) contact hours equals a minimum of twenty-five (25) minutes.
Two hours for renewal. Course must be given by a Florida Board of Occupational Therapy Practice approved provider. Even though you may have been required to complete a 2 hour course in prevention of medical errors for initial licensure purposes (prior to July 1, 2016), you must complete an additional 2 hours of prevention of medical errors for each renewal. Course must include: A study of root-cause analysis; Error reduction and prevention; Patient safety; and Contraindications and indications specific to occupational therapy management including medication and side effects.
You as a licensed professional can make the determination if a course is acceptable for OT continuing education. Subsection (5) of rule 64B11-6.001, Florida Administrative Code, states that courses and programs not approved shall be approved as appropriate continuing education if said course or program meets the criteria as specified in rule 64B11-6.001. The course shall be relevant to the practice of occupational therapy and must be offered for the purpose of keeping the licensee apprised of advancements and new development in occupational therapy, and shall be designed to enhance learning and skills consistent with contemporary standards for occupational therapy practice. The Board of Occupational Therapy Practice and the staff do not approve individual courses.
